In connection with your application for commercial trucking insurance, Torq Insurance Agency LLC ("Company") may procure one or more consumer reports and/or investigative consumer reports about you and any drivers associated with your commercial motor vehicle operation. These reports are obtained for insurance underwriting, risk assessment, and eligibility determination purposes.
Reports may be obtained through any one or more of the following accredited consumer reporting agencies (as defined under the Fair Credit Reporting Act, 15 U.S.C. § 1681 et seq.):

• Inteliifi, Inc. | inteliifi.com
Consumer reports obtained may include, but are not limited to: motor vehicle records (MVR), commercial driver's license (CDL) status and history, FMCSA Pre-Employment Screening Program (PSP) records, driving violations, accidents, license suspensions or revocations, criminal and county court traffic records, identity verification, and other safety-related information obtained from public records or other lawful sources.
You have the right, upon written request made within a reasonable time, to request disclosure of the nature and scope of any investigative consumer report. You may also contact the consumer reporting agencies listed above directly to obtain a copy of your report and to dispute any inaccurate information.
By electronically acknowledging this authorization below, I, the undersigned applicant, acting on my own behalf and as an authorized representative of the commercial motor carrier named in this application, hereby voluntarily authorize and consent to the following:
1. Individual Consent: I authorize Torq Insurance Agency LLC and its authorized agents, underwriters, and carrier partners to obtain my motor vehicle records and consumer reports from any of the consumer reporting agencies listed above, now and throughout the duration of this application and any resulting insurance policy.
2. Employer/Operator Consent on Behalf of Drivers: I certify that I am an authorized representative of the named insured and that I have obtained written or verbal consent from each driver listed on this application to authorize the procurement of their motor vehicle records and consumer reports for insurance underwriting purposes. I understand that each driver's information will be used solely for risk assessment and policy underwriting.
3. Acknowledgment of Rights: I acknowledge that I and each listed driver have the right to request a copy of any consumer report obtained and to dispute any inaccurate or incomplete information directly with the applicable consumer reporting agency.
4. Accuracy of Information: I certify that all information provided in this application is true, accurate, and complete to the best of my knowledge. I understand that any misrepresentation may result in denial of coverage or cancellation of any issued policy.
5. Electronic Consent: I understand and agree that my electronic acknowledgment of this authorization, by checking the confirmation box below, constitutes a valid and legally binding electronic signature under the Electronic Signatures in Global and National Commerce Act (E-SIGN Act, 15 U.S.C. § 7001 et seq.) and applicable state electronic signature laws, and carries the same legal effect as a handwritten signature.
This authorization shall remain in effect for the duration of the insurance underwriting process and any resulting policy period, unless revoked in writing.
